Add one bullet to 'What this Action does NOT do (yet)' to honestly state that historical signal enrichment (churn, hotspot density, prior-incident proximity) does not run in stateless customer-side mode. The decision, policy, and signed attestation are unchanged; only the risk-score amplification is reduced. Closes P1#1 from the 2026-06-04 structural audit.

One bullet added to 'What this Action does NOT do (yet)' to honestly describe what the stateless customer-side run does and does not include: historical signal enrichment (churn, hotspot density, prior-incident proximity) requires persistent cross-run history the CI runner lacks, so it does not amplify the risk score in stateless mode. The decision gate, policy evaluation, and signed attestation are fully functional.
README-only; no action.yml change, no new release. Closes P1#1 from the 2026-06-04 structural audit. Awaiting review.
